The concept of Aeron's character's transformation to a monster was partly inspired by the recent events during the COVID-19 outbreak in Europe. Haga said he had the "feeling of uneasiness... of isolation, and of being just one more thing in the world." In turn, Aeron has a quest to save Elena from the curse and bring her to the sovereign mountain Yoba, but also to find a way to return to a normal life. This theme was described by Haga as "The quest for everything, the attempt of becoming human once again." Also, the game is a part of the "shadow of the pandemic." The game includes elements of adventure, horror, and science fiction: with Aeron and Elena being the main characters, the scope of the adventure is the world of monsters, and the characters' predicament is portrayed by deadly viruses that cause the main character's monstrous transformation. The development team wanted to portray a darker and more terrorizing world. Although Aeron is the protagonist, the game is designed to allow him to interact with other characters and the environment in an undefined manner.[18]

During the course of development, there were some concerns with so few monsters and preparing the game for a possible "very large" number of players.[4] These concerns were addressed by increasing the number of monsters, and making the enemies more dangerous.
